1. Purpose of Use of Personal Data

Personal data provided to United Semiconductor Japan Co., Ltd. (“USJC”) will be used for the following purposes. In the event you and USJC separately agree upon different purpose of use, use of your personal data will be in accordance with the scope of such other purpose.

  4. Procedures for requesting disclosure etc.

    When making following requests with respect to the personal data you provided to USJC, please contact the USJC staff that you provided your personal data with: ① notification of purpose of use, ② disclosure of personal data retained ③ disclosure of record of personal data provided to third party, ④ correction, addition or deletion, or ⑤ erasing personal data, ceasing to use or disclose personal data to third party.
    If you are unsure of the identity of said USJC staff, please follow the instruction below.

Instruction of Request

First of all, please ensure to inform the “Support Desk” as provided above in 2-(3) by phone. As next step, please provide “request of personal data” as specified under (2)(i) below to the mailing address listed as following. Regarding the aforementioned request of personal data, no specific format is required as long as all the necessary information listed under (2)(i) to (iv) is clearly stated thereon. In the event inquirer's request is for “disclosure of personal data retained” or “disclosure of record of personal data provided to third party”, please also enclose postage stamps with face value equaling to JPY1,000 as handling fee.

    Request of personal data (must be made in writing but no specific format is required)

    Please include the following information on your written request.

    • Requesting date.
    • Name and address of the inquirer then affixed with inquirer's personal seal.
    • If the inquirer is under 16 years of age, the signature and seal of his or her legal guardian.
    • If the request is made by a duly authorized representative on behalf of the inquirer, the written request shall include representative's name and address; and affixed with representative's personal seal.
    • Contact information (telephone number available during daytime on weekdays) and e-mail address of the inquirer.
    • If inquirer's information provided to us (name, address etc.) differs from the above, please inform us of this fact and the information so provided previously.
    • Details of the request (whether it's for “notification of purpose of use”, “disclosure of personal data retained”, “disclosure of record of personal data provided to third party”, “correction, addition or deletion”, “erasing personal data, cessation of using or disclosing personal data to third party” and specific details of the inquirer's request).
    • Reasons of the request (please explain as specific as possible).
    • If the request is for “disclosure of personal data retained” or “disclosure of record of personal data provided to third party”, please specify the method of our reply from the following options: “by email enclosed with electromagnetic record”, “by post mail enclosed with electronic media containing the electromagnetic record” or “by post mail with record on paper base”).

    When the request is made by inquirer directly.

    • Please provide the specified identification information listed in either A or B.
    • Please ensure the identification document provided to us is the same name and address as stated on the request.
    • Please redact information of domicile, if any.

    All documents listed below are required.

    • Power of attorney sealed by the inquirer together with seal registration certificate of the inquirer.

    ※If the request is made by the parent having or granted with custody or the legal guardian on behalf of a minor, power of attorney is not required on the condition that relevant documents proving the parental relationship or grants of custody is provided. Aforementioned documents include but not limited to a certified copy of household registration transcription or the extract thereof, or the certificate from the family court etc.

    Handling fee

    • If inquirer's request is for “disclosure of personal data retained”or “disclosure of record of personal data provided to third party”, please also enclose postage stamps with face value equaling to JPY1,000 as handling fee.
  3. Remarks on Requests for Disclosure

    • In the event USJC is not obligated to respond as in accordance with Act on the Protection of Personal Data, or if the necessary documents or information of the request of disclosure are incomplete, USJC may not be able to respond to inquirer's request.
    • USJC will use the documents provided together with inquirer's request of disclosure only to the extent necessary to respond to such request. Please note the documents provided to us will not be returned.
    • In the event USJC cannot legally respond inquirer's request for “disclosure of personal data retained” or “disclosure of record of personal data provided to third party”; or in the event inquirer's request is for “notification of purpose of use”, “correction, addition or deletion of personal data” or ” erasing personal data, ceasing to use or disclose personal data to third party”, USJC will reply by e-mail to the e-mail address of the inquirer (or the inquirer's representative).
      In the event inquirer's e-mail address is not stated on the request, USJC will reply to the inquirer (or the inquirer's representative) by post mail sent to the domestic address stated on the request.
    • Upon deleting or erasing the personal data as in accordance with inquirer's request, USJC will retain a copy of the request, all documents of the inquirer's identification and our response made in writing for archival purpose.
    • Depending on the details of inquirer's request, we may not be able to provide our response timely.
    • Please note that in the event inquirer's personal data is deleted, erased or ceased to be provided to third party as in accordance with the request made, inquirer may not continue to use or access the service provided by USJC with respect to personal data.
